Restylane: Manufactured by Medicis Pharmaceutical Corporation, Restylane is made from hyaluronic acid, one of the structural components of skin noted above. It is considered a semi-permanent filler and will last six to twelve months. Medicis claims that Restylane is the best selling dermal filler in the world.

Juvéderm: Manufactured by Allergan, Inc. (the makers of Botox) Juvéderm is also a gel made from hyaluronic acid. According to Allergan’s website Juvéderm is the ONLY hyaluronic acid (HA) filler FDA approved to last up to 1 year and is the first hyaluronic acid dermal filler that the Food and Drug Administration (FDA) has approved as being safe and effective for persons of color.

Radiesse: Manufactured by Bioform Medical Inc. Radiesse, formerly known as Radiance, is composed of calcium based microspheres suspended in a water based gel. According to the manufacturer Radiesse stimulates the body to produce new collagen in a process called collagenesis. Bioform has clinical data to support their claim that Radiesse may last more than two years.

Youthful Hands

Botox and Dermal Fillers: The Perfect Combination

At Epione we consider patient education one of our primary purposes. We believe that one key to patient satisfaction is aligning the results we can offer, and do our best to achieve, with the expectations of our patients. Patient education is fundamental to this relationship. And an educated patient typically has a better understanding of the post-procedural at-home aftercare that is often key to maximizing their results.

Dr. Ourian is a big proponent of combining Botox injections and dermal fillers such as Radiesse, Restylane and Juvederm to achieve optimal results for the patient who wishes to address the wrinkles on her face and obtain a more youthful look.

Why Botox AND dermal fillers? It is important to understand and distinguish between two types of facial wrinkles: dynamic and static. Dynamic wrinkles occur in people of all ages. The creases that you see between and around your eyes when you squint are examples of dynamic wrinkles. They appear temporarily when a muscle contracts and causes the overlying skin to crease like an accordion. When Botox is injected into a muscle, it blocks the nerve impulse from reaching that area, and as a result, the muscle weakens. As the muscle weakens, the skin overlying the muscle relaxes and the wrinkles in the skin gradually soften and often disappear.

Static wrinkles are always visible, even when all the facial muscles are at rest. Static wrinkles cause the deep grooves between the nose and mouth known as naso-labial folds, or smile lines, and tend to develop in skin that has thinned and stretched as a result of the aging process. Dr. Ourian uses dermal fillers to plump up the groove from beneath.

There are even some anecdotal reports suggesting that Botox helps slow down the breakdown of the filler due to less movement in the area by reducing muscle contractions.

Injectables: M.D. vs. R.N. – Is Good Business Good Medicine?

We find three recent trends disturbing:

  1. Some physicians are turning to nurses to perform most of their injectable treatments. Reasons given for doing so include increasing practice revenue streams and career advancement for the “nurse injectors.” At Epione only physicians administer Botox and dermal fillers such as Radiesse and Juvederm.
  2. Nurses setting up independent cosmetic practices, sometimes asking physicians to sponsor them. A nurse injector is the term for a registered nurse who provides cosmetic injectables treatments. Some states restrict or do not allow the practice of nurse injectors. In California nurses can inject Botox and dermal fillers only under the direction and supervision of a physician. Only a physician can prescribe medication including Botox and dermal fillers. And only a physician can legally evaluate conditions, recommended treatment, offer alternatives and clearly defined the potential outcomes including any complications.
  3. Mall and chain-store med-spas. Of particular concern are nurses who administer injections at medical spas where physician supervision may be minimal to non-existent. It is often the case that one physician is assigned (read “paid”) to supervise a group of such spas despite the physical impossibility of he or she doing so. Who is there to guaranty the setting is a proper, medically-equipped office, with safety and sterilization procedures? Is there a physician on site in the event of a complication or emergency?
